A: Although you are not responsible for verifying the accuracy of the information on a W-4, you may not accept an invalid W-4 from an employee.  If an invalid form is presented, you must ask the employee to complete a new form.  If the employee refuses to present a new, valid W-4, you must either keep withholding according to the most recent valid W-4 form on file for the employee, or if the employee has no valid W-4 on file, withhold at Single and Zero allowances.
